Total Hip Arthoplasty Using Hydroxyapatite-Coated Smooth-Surfaced Anatomic Femoral Stem

Abstract
Purpose: To evaluate the minimum 7 years results of a total hip arthroplasty using a hydroxyapatite (HA) coated smooth-surfaced anatomic femoral stem.

Materials and Methods: Between November 1992 and June 1995, 62 total hip arthroplasties were performed in 54 patients, using a HA coated smooth-surfaced anatomic femoral stem. Of these, 32 hips, in 28 patients, with a minimal 7-year follow-up, were included in our study. The mean follow-up period and age at the index operation were 94 months and 49.5 years, respectively. The clinical results were evaluated using the Harris hip score, and the incidence of thigh pain was also checked. The radiological evaluation was performed using parameters, such as migration, radiolucency, cortical hypertrophy, calcar atrophy and osteolysis of femur.

Results: The mean preoperative Harris hip score of 54.2 points increased to 92.5 points after one year, but had slightly decreased to 88.5 points at the last follow-up. The incidence of thigh pain was 6.3% (2 hips) at one year, but this increased to 12.6% (4 hips) at the last follow-up. Stem migration, radiolucency, cortical hypertrophy and calcar atrophy occurred in 2 (6.3%), 4 (12.6%), 9 (28.3%) and 6 hips (18.8%), respectively. Femoral osteolysis developed in 6 hips (18.8%), with 3 (9.4%) requiring revision surgery.

Conclusion: The clinical results of a total hip arthroplasty, with a minimal 7-year follow up, using a hydroxyapatite-coated, smooth-surfaced anatomic femoral stem were successful. However, the high rates of femoral osteolysis and revision are problems that require a solution.
